Type 2 Diabetes Program

The Type 2 Diabetes Screening & Education Program helps individuals who are at risk of developing diabetes or have been diagnosed with the disease. Support is done through 1. Community outreach and screening, 2. Preventive services which increase early identification and promote healthy habits, and 3. Intervention services that provide education and support.

The program is offered by four members of the Chronic Disease Management Consortium; California Hospital Medical Center, Good Samaritan Hospital, Huntington Hospital, and the National Health Foundation (NHF). Preliminary program data analyses show a 70% increase in the number of participants who reported checking their blood sugars daily, almost a 35% decrease in the number of participants who reported elevated Hemoglobin A1cs, and increased confidence in the following areas 1. diabetes care management (75%), 2. healthy eating (61%), and 3. exercise habits (47%).
